Make an Appointment

If you are a New Client, please fill out New Client Paperwork. Once we receive your submission, we will contact you to schedule your first appointment, and you will get an email from our client Portal, inviting you to enroll in our online system where you can sign required documents, request appointments, and manage your bills.

If you are a Current Client, please contact your therapist directly, or log in to your secure Client Portal to request an appointment.
